 | Why keep appointments booklet The appointments booklet is a simple educational tool for giving to patients just starting out on their care, designed to answer questions about why they should keep recommended appointments. The booklet takes the format of a small greetings card with an additional tear of section for listing the patient’s first 12 appointments on one side and a referral request statement on the other. Inside the booklet answers questions from a patient’s viewpoint. Why so many treatments? The pain has gone so what are you treating? Why can’t I miss the odd appointment? Why should I let you know if it’s impossible to attend my appointment? On the rear, there is a simple to understand yet detailed explanation of how Chiropractic works so enthusiastic patients can fully understand why success is achieved via regular treatments.

 | Patient Record Booklet A twenty four page A5 portrait style booklet available in either all white or with a green outer sheet to distinguish between different Chiropractors. Designed to fit in a regular filing cabinet, either two rows down the length or three across. Designed by Chiropractors to encapsulate all aspects of patient records regularly needed in the practice. The booklet is based around the concept of the “travel card” used by Chimes Consulting and so will fit happily into practices using this technique as well as those who are not. The booklet contains… Front cover records patient name, date of birth and unique pt number 1 page of contact info, GP details + general info such as occupation. 1 page for consent for consult, exam, treatment and 10 re-consents 8 pages in grid format for consult and exam notes + 10 re-exams 1 page for patient profile detailing, smoking, drinking, medication etc 9 pages of ruled space for regular visit note taking 2 pages for treatment plan and appointment recording Back has dermatomes map, exercises given , diagnosis & x-rays taken.
